AGREEMENT FOR PROGRAM PARTICIPANTS
Your mentoring partnership is about to begin! Step one of the contract includes the "Terms and Conditions." You must agree to these conditions in order to continue with your partnership. GROUND RULES:

The expectation is that mentors will be called upon for two hours per month by mentees who need counsel and guidance. The mentor and mentee will decide who initiates the contact and when the meetings are to take place.

Both the mentor and mentee must agree to keep all information discussed strictly confidential.

Both parties are aware that any expectations or discussion of future or current career opportunities with the mentor or mentee's company are inappropriate and are prohibited during the duration of the mentorship program.

If the mentor and/or mentee are uncomfortable with the program relationship, they should contact NAMIC's national office as soon as possible to resolve the situation.

Both parties agree to fully participate throughout the nine-month period and are unaware of any circumstances that would alter the mentorship experience for any significant amount of time (e.g., extended vacation of more than three weeks, leave of absence).
Participation of both mentors and mentees in the L. Patrick Mentorship Program is strictly voluntary and without compensation. A mentee is solely responsible for evaluating any opinion or advice received from a mentor, as well as for any decisions or actions taken or not taken because of that opinion or advice.
Mentors and mentees will be permitted to participate in the L. Patrick Mentorship Program only upon acknowledgement and acceptance of these parameters of voluntary participation and individual responsibility as indicated by continuing to the next page.
